NOTICE:Multi-Plan and Three Rivers plans are for OUT of AREA USE ONLY; that is, for use BEYOND the 75 mile radius of Kingston. The provider will be treated as OUT of NETWORK if either plan is utilized for services within the 75 mile radius of Kingston. If you need assistance with a provider within the 75 mile radius, contact NHAI at 1-888-679-2400. An important notice regarding new PPO rules can be found under Amendments-Modifications.
FRAUD-You are committing a fraudulent act against your plan when: 1.-You fail to inform the plan and/or providers of OTHER MEDICAL/RX COVERAGE, including MEDICARE; 2.- You fail to enroll in MEDICARE PARTS A and B when first eligible, including eligibility due to disability.; 3.- You add a dependent to your coverage who is not a legal dependent. Example: You represent that you are married when you are not; you claim a dependent that is not your child as with a step child for whom you are responsible for providing coverage; or any other dependent for which you do not have legal custody.; 4.-You fail to inform the plan of a change in family status, such as legal separation, divorce, birth, death, domestic partner dissolution, etc. Members are responsible for knowing the plan rules. NOTICE:When you commit fraudulent acts, you are personally liable for reimbursing the plan for all charges paid by the plan that would not have been paid had you timely notified the plan of the event.
MEDICARE: no longer covers Diabetic supplies under Part B. They are now part of the Rx benefit.
OUT of AREA STATUS: Coverage will be effective as of the date received of all necessary form information. This is especially important for students who need out of area coverage.
MATERIAL MODIFICATIONS: PLAN CHANGES-Eff. 1/1/2012 The following modifications to the Kingston Trust Fund were approved by the Board of Trustees at their quarterly meeting on September 27, 2011 and will become effective on Jan.1, 2012. The changes were necessitated by the increased use of the plan resulting from the federally mandated inclusion of both children to age 26 and 100% covered preventative procedures and the expensive protein-based macro chemotherapy drugs. They are made as a part of the Trustees fiduciary responsibility.
Medical: Increase regular office and Mental Health office visits from $15 to $20; Any non-NHAI provider is considered to be an Out of Network provider. Increase Multi-plan, Three Rivers, and any other Out of Area co-pay from $20 to $25; Increase Lab tests, (billed separately from an office visit), from $15 to $20. Rx: Increase Brand Name Retail co-pay from $30 to $35.

MEDICARE: Enrollment information, including the new necessity of a physical address when a PO Box is used. Go to: Trust Reports, Medical, and read beyond the Coordination of Benefits chart. Also, if you are a retiree who has or will delay Social Secirity payments until full retirement benefit age, you still must enroll in Medicare Parts A and B at age 65. Rx Costs: Local pharmacies vary,greatly,in the costs they charge for the same prescriptive medicines. Where you choose to fill your local Rx can save the Trust money. Check NYS prices, by county, at: www.Rx.NYHealth.gov. It's an eye opener!
NHAI EXTENDS DONATION TO KTF FOOD BANK
Joseph and Rosalie Fiegoli of NHAI have extended, permanently, their donation to the KTF Food Bank. A contribution of $500/quarter will be made to the Food Bank that was established last year by KTF to aid children and families in our school community.
CURRENT REMINDERS
With the passage of the Mental Health Parity Act earlier this year that mandated medically necessary physical health and mental health/behavioral benefits must be of equal benefit, a reminder that all physical treatments (chiropractic, massage, acupuncture, etc.) and mental/behavioral (therapists, counselors, social workers, etc.) are limited to six (6) initial visits. All visits in excess of six (6) must be precertified.
The new dependency coverage rules for 19-26 year olds state that coverage will end at the end of the month in which the dependent turns 26.
Please be sure to keep a current credit card on file so that your MEDCO co-pays can be immediately charged to your account in order to expedite the filling of any mail order Rx.
ENROLLMENT UPDATES: Every member has an electronic enrollment form on the NHAI website. Simply put in your ID and password. Make all changes on the site. If you need assistance, contact the Trust Office.
Status changes include name, address, phone/email, and marriage, separation, divorce, newborn, retirement, student/non-student dependent, and Medicare eligibility.
Remember, to receive some program services, such as diabetes, weight loss, Healthy Beginnings maternity, and infertility require an enrollment. Consult the plan book for additional information.
